Success Story: Addressing Social Determinants of Health for Pregnant Member Improves Access to Prenatal Care

Pregnant Female A Passport RN Care Advisor was working with pregnant Passport member, Sonia*, who had major transportation barriers, asthma, and housing needs. Sonia had missed at least one prenatal appointment related to this transportation barrier and was requesting housing resources/assistance because she had to break her lease at previous apartment due to an infestation.

Sonia had a safe place to stay temporarily at her mother’s house, but wanted to re-establish independent housing. Our RN Care Advisor sent a referral to a Passport social worker for housing resources and assistance. She also referred Sonia to the Kentucky Federated Transportation Services of the Bluegrass (FTSB) for transportation assistance.

She educated Sonia on the importance of continued prenatal care and signs/symptoms of when to call her OB or seek care.

As a result of these efforts, Sonia was able to secure independent housing with assistance/resources and is happy and safe at her new apartment. She has not missed any more prenatal appointments since receiving referral for transportation assistance!
