A phone call from us to personally welcome you to the plan and answer any questions you may have.
Now, it’s time to start getting the most from your Passport coverage! We know that using your benefits can be confusing at times, but here’s where it gets a little easier. Use this checklist to take charge of your health and your family’s health from the start – and get the most out of your benefits!
It’s easy to get started as a new member. Just take these 7 Simple Steps:
1. Know the name of your Primary Care Provider (PCP)
Check your member ID card for the name of your PCP. If you want to change your PCP, you can do this by calling 1-800-578-0603.
2. Schedule your first PCP visit
You’ll want to see your PCP, even if you are not sick, to get set up as a new patient. Your PCP needs to get to know you and your health history. The more your PCP knows, the better he or she can help you.
3. Learn about your pharmacy benefits
Know what copays you have for your medicines. If you have questions or concerns, call us at 1-800-578-0603.
4. Save the 24/7 Nurse Advice Line phone number
Store 1-800-606-9880 in your phone or post it where you can see it. Call the nurse line anytime of the day or night. You can call when you have any medical question or for help with any symptoms you are having.
5. Fill out your Health Risk Assessment (HRA)
Your HRA form tells us how we can meet your needs. Fill out your form and send it back to us in the post-paid envelope that came with your new member packet. You can also call us at 1-877-903-0082 and we can help you fill out the form over the phone.
6. Set up an account on our member portal
Sign up on our member portal at www.passporthealthplan.com/memberportal to print a temporary ID card and keep track of your health care. You can also check “yes” to receive Passport texts and emails. You’ll be the first to know about any changes and get key information about how to get the most out of your benefits!
7. Call us with questions about how to better manage your health
If you have questions or concerns about how you can better manage your health, please call us at 1-877-903-0082. We’re happy to work with you one-on-one!
