The new program will be called Kentucky
HEALTH and it will begin soon.

4 Simple Things You Can Do NOW:

Make sure your address is up-to-date with the Department for Medicaid Services (DMS). It’s very important that you’re getting your mail!

Kentucky HEALTH (Helping to Engage and Achieve Long Term Health) is a new way to do Medicaid. It offers health insurance for certain low-income adults and their families. It encourages people to do things that lead to better health and well-being.

Make sure your address is up-to-date

You can double check or change your address by calling DMS at 1-855-459-6328 or by calling one of these offices:

• If you applied for Medicaid at the Department for Community Based Services (DCBS), call

• If you applied for Medicaid at the Social Security Income (SSI) office, call 1-800-772-1213.

• If you applied for Medicaid at, call 1-855-306-8959. You may also report address changes at
