The Passport Foundation develops initiatives and partnerships beyond clinical care to improve the health and well-being of all Kentuckians.

 


Our Focus

Good health has long been defined as simply not being sick, yet health and well-being are also strongly influenced by complex social factors such as where we live, how we work, the safety of our neighborhood, and the resilience of our families. In fact, research indicates that traditional clinical care accounts for just 20% of health outcomes.

This growing recognition of the impact of social determinants of health has begun to fuel innovation. New approaches are starting to address what happens outside traditional medical systems with a focus on the most vulnerable to risks.

Across the state of Kentucky, The Passport Foundation sees a drastic cut in Medicaid funding. It focuses on supporting programs and services that improve social determinants of health for people in underserved and low-income communities.

Social Determinants of Health the Foundation focuses on:

  • Workforce Development
  • Transportation
  • Housing
  • Food access & nutrition education
  • Mental health and substance abuse recovery

 


About Us

The Passport Foundation was created in 2017 to assist under-served and low-income communities, throughout Kentucky to help people of all ages live healthy lives.  We do this by securing and providing funding for various initiatives and meaningful partnerships with service providers, corporations, community health organizations, other foundations, and educational institutions.

A Message From the Executive Director, Keisha Deonarine

I am incredibly humbled and honored to serve as the inaugural Executive Director of the Passport Foundation. The Foundation secures and provides funding to develop initiatives and partnerships beyond traditional clinical care which strive to improve the health and well-being of all Kentuckians. It is not something I take lightly, and I promise to lead with hope, commitment and promise.

What appealed most to me about this opportunity is the change – be it social, economic, or environmental – that I will help impact unserved communities in Kentucky; communities that are comprised of people who struggle daily to live healthy lives.

With my extensive background in finance, economic development, innovation, and health strategies, this job fits me perfectly. We want to focus on “root causes” of health inequities such as workforce development, transportation, housing, food access, and others that we know can change people’s ability to improve their health. I want to look at fundraising in a new way and have our donors, contributors and sponsors see first-hand all of the changes that they can affect and be part of the dynamic work the Passport Foundation is undertaking.

My parents, sister and I moved to America when I was just a little girl and our lives were hard for many years. I remember living humbly in New York as my parents struggled to see where our next meals would come from. At the time, I thought the world was only as big as what was around us.

Now, I think back on the hard work my parents endured and the dedication they had to wanting more for my sister and me and I realize how it really does take a village. When I see my fellow Kentuckians needing help, I often think about the times I wished someone would help my family. It is my honor and privilege to serve the Passport Foundation and I will do this work with integrity, pride, compassion, and love.

My hope is to lead the Passport Foundation into many years of revenue growth and stability. My commitment is to lead with strong morals and integrity. My promise is to spend every minute I can to change the lives of under-served people and communities across Kentucky.
