Our Medicare Plan

Passport Advantage is a Medicare Advantage plan for Passport Health Plan members with Medicare Parts A and B. We are a health maintenance organization licensed by the Commonwealth of Kentucky with a Medicare Advantage contract with the Centers for Medicare and Medicaid Services (CMS).

Only certain people can enroll in Passport Advantage. You must be a Passport Health Plan member with Medicare Parts A and B to enroll in Passport Advantage. You also must live in one of the counties in our service area: Jefferson, Oldham, Trimble, Carroll, Henry, Shelby, Spencer, Washington, Marion, Nelson, Larue, Bullitt, Hardin, Grayson, Breckinridge, or Meade.

At the end of any calendar year, Passport Advantage could choose not to renew its contract with the Medicare program. In addition, CMS could cancel its contract with Passport Advantage at any time. The Medicare program could also reduce the number of counties we serve. If our contract with CMS were to end, you would be disenrolled from Passport Advantage. However, you would be able to receive your Medicare benefits through:

  • Original Medicare and a Medicare Prescription Drug Plan (PDP), or
  • Another Medicare Advantage plan.

If our contract with Medicare ends, you would continue to receive your Medicaid benefits through Passport Health Plan.
