2008 KCHIP Initiative
In September 2008, Governor Steve Beshear announced a new initiative to enroll approximately 35,000 children into KCHIP by 2010. In order to reach this goal the application process was simplified by eliminating the face to face interview and implementing a new mail in application process.

The mail in application is a six page document which requires the applicant to provide proof of income, citizenship, identity and proof of daycare expenses. The application can be downloaded from the KCHIP website and either mailed or faxed with appropriate information. The applicant will be given up to 30 days to process DCBS will begin accepting the new application beginning November 1, 2008. Face to Face interviews will be accepted at the local DCBS offices if applicants desire to meet with a caseworker.


KCHIP
is the state of Kentucky's version of a federal program to ensure that children who do not qualify for traditional Medicaid, but remain uninsured, have access to comprehensive health insurance.

If your child is eligible for KCHIP and lives in our service area, he or she becomes Passport HealthPlan Member.

As a single dad, I am so relived to know that my kids are taken care of through KCHIP. They receive excellent care and are able to grow and stay healthy.
My kids have been on KCHIP for 8 years now and without it I don't know what we would do. One has asthma and the other has diabetes, I am so thankful for the good care they are receiving and the information I receive to help them control their diseases.
