New Changes When Renewing Your Medicaid Benefits

You must now have proof of citizenship and identity when renewing your Medicaid benefits. This is a federal law that includes Passport Health Plan and KCHIP members. The law went into effect on July 1, 2006.

When you go to renew your Medicaid benefits.
The Department for Community Based Services (DCBS) in your county will need you to bring one of these:

• A U.S Passport used for travel outside the country
• Certificate of Naturalization
• Certificate of U.S Citizenship

If you do not have the documents above, one of the following documents will be accepted, along with a picture ID:

• U.S. Birth Certificate* (only if born outside of Kentucky)
• Certification of Birth issued by the Department of State
• Certification of Birth Abroad
• Report of a Birth Abroad of a U.S. Citizen
• U.S. Citizen I.D Card

*All documents must be originals.

*If you do not have these, ask your DCBS caseworker for help. There are other documents that can be used. Your caseworker may be able to get these records if you were born in Kentucky. If you have a mail-in recertification, you can send your proof and recertification paperwork or you can bring it to your caseworker.
