As the Director of Community Engagement for Passport Health Plan (a Medicaid managed care plan serving over 310,000 Kentucky members since 1997), Dana is responsible for oversight of all community outreach initiatives, including health education and health equity, diversity, and inclusion services.  To successfully reach and ensure that our members have the tools they need to improve their health and quality of life, Dana manages a staff of individuals who reside in locations across the Commonwealth. This talented team provides outreach and education to members, potential members, advocates, caregivers and community partners related to Passport’s benefits and services.

Prior to Passport Health Plan, Dana earned extensive experience in the healthcare industry with over 30 years’ experience in commercial, Medicare and Medicaid lines of business, including 17 years with Anthem Blue Cross and Blue Shield.  Her passion to serve the community is exemplified by her history of volunteerism in the community and her current and previous service on several boards and committees, including the following:

  • Board of Directors: The Salvation Army Advisory Board, Center for Accessible Living, Elderserve, Louisville Metro’s RSVP/Foster Grandparents Advisory Board, Kentuckiana Regional Planning & Development Agency (KIPDA)’s Long Term Care Ombudsman Advisory Council.
  • Associations: Community Health Worker Association; Greater Louisville Aging in Place; Greater Louisville Association of Underwriters (GLAHU); and TRIAD.
  • Other Community Involvement: Volunteers of America, Metro United Way, KIPDA, Lincoln Trail District Area on Aging, various community action agencies, and TRIAD.

Dana is a resident of Louisville, KY, where she lives with her two dogs and cares for her father.

Kristin’s main role is managing the schedule for the Director of Community Engagement and assisting and supporting Passport’s Community Engagement team. Kristin supports improving diabetes care, bringing awareness to pancreatitis, rheumatoid arthritis and kidney disease. Kristin is passionate about family and faith, singing in her spare time, and supporting her Community Engagement team.

Kristin has spent the past 25 years working with various healthcare companies throughout the Louisville area.

Marji has spent 10 years as a Passport Community Engagement Rep in the Louisville and surrounding counties. She is passionate about her family, her work family and friends, her pets, music, our communities, most holidays, and trying to be as green as she can. She believes in health and wellness, along with treating people and animals with compassion and fairness, education and the arts. She has served on a variety of boards and committees, including several Chamber of Commerce board and committees, various school Family Resource Youth Service Center Advisory Boards, and more.  Marji also supports many causes and organizations including Fund for the Arts, American Heart Association, Metro United Way, St. Jude Children Research Hospital, the Kentucky Humane Society, Harbor House, the Homeless Coalition, and others.

Before working at Passport, Marji enjoyed a long career in radio and TV promotions.  She was the first person in Louisville to receive an award from American Women in Radio and Television for Promotions and Marketing, and most recently made the Dean’s List when she went back to college.  Marji has more years of experience than she wants to admit to.

As a Community Engagement Representative for Passport Health Plan, Priscilla Schwartz has over 29 years of experience providing services to the Medicaid population in Kentucky.

Priscilla received a Bachelor’s Degree in Communications from the University of Kentucky and a Master’s Degree in Mental Health Counseling from Western Kentucky University.  Beginning her career at Adanta Behavioral Health, she served as Prevention Specialist, IMPACT Coordinator and School Based Therapist.  She then served Monroe County Schools as a counselor and Cumberland County Schools as the Family Resource and Youth Services Center Coordinator, serving on the state FRYSC Board.  Priscilla launched into Passport Health Plan in the fall of 2017 where she serves members and advocates in South Central Kentucky.

Raised in Monticello, Kentucky and living 24 years in Burkesville, she became a Bowling Green resident in July of 2017.

Priscilla is an enthusiastic member of the community and enjoys working with our members by helping them become healthier Kentuckians.

As a Passport Community Engagement Rep in the Louisville and surrounding areas, Joy is a valuable resource for our members and advocates. She collects information in each of her counties to have if needed when she is out in the field.

Joy is passionate about children and the elderly. She volunteers at her church to work the nursery. She loves spending time with her grandkids, UK Basketball and visiting the many wineries in the area.

Joy has over 40 years in the medical insurance field with various Insurance companies. She has also worked in the dental and home health fields.  She serves on the Managing Board of the Neighborhood Place and is on many School Advisory Councils and committees.

Stacy is a Community Engagement Rep for the Eastern part of Kentucky. Before coming to Passport, she spent two years working in public service with AmeriCorps.  Stacy has also worked in elementary education as a substance abuse course educator for grades K through 6 with Operation UNITE in Eastern KY.  She also has a background in radio as an on air personality and commercial voiceover.

Stacy supports Eastern Kentucky schools in extra-curricular activities.  She believes that having constructive ways to spend their time is crucial to the future success of today’s youth.  Stacy is passionate about helping others, especially mothers suffering from drug dependence. She has collaborated with area agencies and a local hospital to form the Big Sandy NAS (Neonatal Abstinence Syndrome) Coalition. This coalition provides education for the Big Sandy area about NAS and how to prevent it.

Stacy is a lifelong resident of Johnson County where she has raised her three children with Noel, her husband of 24 years.  They both graduated from Johnson Central High School where he is now principal.

As a Passport Community Engagement Representative in Eastern Kentucky, Zach is passionate about helping others and being an advocate and voice for his area. He believes in treating people the way you would like to be treated, leaving this world a little better than you found it, and that little acts of kindness will change the world.

Zach has more than six years of experience in the health insurance and outreach fields. Prior to Passport, Zach spent three years as an Application Assister and Outreach and Enrollment Specialist for the Kentucky Department for Medicaid Services (DMS). During this time, he enrolled more applicants in Medicaid and private insurance than anyone else in the state.

Zach has many roles and titles, but the two he is most proud of is Father and Husband. Zach’s favorite hobbies are cheering on his favorite sports teams and weight lifting.

Sandra is passionate and intentional about the acceptance and inclusion of all people.She has over 25 years of experience in various fields all involving Community Engagement. Sandra has spent her entire career working with and advocating for undeserved and underrepresented communities.She serves as the bridge that connects the people in these communities to the services and resources they need.

As Equity, Diversity and Inclusion Program manager and subject matter adviser on the delivery of culturally responsive services, Sandra promotes and fosters Passport’s mission, vision, and values both internally and externally. She identifies opportunities that improve the health of undeserved groups, members with communication needs, and the disabled. She is responsible for ensuring that employees working for Passport are informed on Title VI of the Civil Rights Act of 1964, the Enhanced Culturally and Linguistically Appropriate Standards (CLAS), the Americans with Disabilities Act (ADA), and Section 1557 of the Affordable Care Act (ACA). She is bilingual in Spanish and English.

In her role, she is responsible for statewide strategizing, creating, and executing educational and outreach opportunities to promote and improve the health and quality of life of our members. She contributes to the efforts of establishing, maintaining, and enhancing relationships with various service agencies, associations and business groups.

Carolina believes everyone has gifts and talents that can make a lasting impact and to live a life of significance. Her passion for helping people fuels her work. She works closely with underserved groups, members with communication needs and the disabled.She is bilingual in both English and Spanish.

Lisa is passionate about health and wellness. She loves motivating people to be the best version of themselves. She likes to educate people about how social determinants of health make an impact on the choices people can make.  Lisa has a Master’s degree in health and leisure education and is a certified personal trainer. Before coming to Passport, Lisa worked in a variety of roles in different industries like an account manager at Carewise Health in Louisville, fitness manager at 24Hour Fitness in California, Program Director at ResCare HomeCare in Arizona, Senior Campaign Director at The Leukemia & Lymphoma Society in Louisville, and trainer and program manager of Training for the Future at Boston University. She has brought a health & wellness perspective to all of these different roles.

Lisa runs Passport’s LIFE (Lifestyle Improvements For Everyone) program and is available to come talk to your group about a variety of health topics (please see the LIFE Program brochure).  In her free time, Lisa enjoys being active and outdoors and likes the challenge of participating in Spartan races.

A big believer in healthy communities, Zach Osborn supports working together to improve the health and lives of all involved. Zach Osborn is passionate about getting people the knowledge to help them be their best self.

Zach is no stranger to working with people, having spent 2 years as a health educator and HMR specialist. As health educator, he helped to educate people on health, exercise, and nutrition. In his previous position as HMR specialist for a different organization, he helped coach people to lose weight. His biggest success was helping a man lose over 100 lbs.

Zach has more than 2 years of experience.

Zach lives in the small town of Debord KY with his wife Emaleigh and chocolate lab Auggie.

As the Community Engagement’s Operations Manager, Michele supports and motivates team members to reach out and think outside the box to meet and exceed goals, while at the same time assisting those in our communities. Michele is passionate about family/loved ones, which extends with a domino effect to a multitude of individuals.

Michele has spent approximately 35 years in health-related positions, including a position to create initiatives addressing and reducing the uninsured population nationally. She has served on the Family Health Centers Board of Directors for over 10 years.

Tonnie assists Passport members and the entire Community Engagement team from our office in Prestonsburg, KY. She is always ready and eager to offer a helping hand and greet you with a smile.

Tonnie supports education and growth, and is alwayslooking for any opportunity to learn something new. She is passionate about the value of family, being courteous to others, and acts of kindness, no matter how small.

Before Passport, Tonnie spent 11 years as a Respiratory Therapist (RT) and eight of those years also as a Front Office Coordinator. As an RT, Tonnie was able to treat and care for patients. She provided a broad range of care from nebulizer treatments and arterial blood gases to ventilators and trach care. As a Front Office Coordinator, she was able to supervise coworkers, assist patients, learn and work with the various types of insurances, etc.

Tonnie resides in Johnson County, Kentucky with her husband Brent of seven years. She has a college-aged son, Logan, and 1st grade daughter, Tatum. Tonnie also has three fur baby cats: Buttons, Luna, and Star. Tonnie is a huge Louisville Cardinals fan and enjoys watching sports. She also enjoys family time, shopping, being outdoors soaking up the sun by the pool and lazy days at home doing absolutely nothing. No matter what she is doing, she is always grateful and makes the most fun out of it!
