Passport Health Plan: At-A-Glance
Passport Health Plan is the operating name for University Health
Care, Inc., a Medicaid managed care plan that serves the Medicaid
and KCHIP populations in the Commonwealth of Kentucky. The Plan
was established in 1997, by a group of providers with a history
of caring for Medicaid members. Those providers include University
of Louisville Hospital, Jewish Hospital, Norton Healthcare,
and the Louisville/Jefferson County Primary Care Association,
which includes Jefferson County Health Department and Louisville's
two federally qualified health centers, Family Health Centers
and Park DuValle.
The Plan serves approximately 140,626 members in the Commonwealth
of Kentucky, which is comprised of the following 16 counties: Jefferson,
Oldham, Trimble, Carroll, Henry, Shelby, Spencer, Bullitt, Nelson,
Washington, Marion, Larue, Hardin, Grayson, Meade, Breckinridge.
Passport Health Plan is administered by AmeriHealth Mercy, a multi-state
Medicaid managed care organization headquartered in Philadelphia,
Pennsylvania. AmeriHealth Mercy and its affiliates have nearly 20
years of experience and expertise exclusively serving Medicaid and
CHIP populations.
